Spine fracture Treatment in Delhi

A spinal cord is an extension of the brain that is present in the spinal canal. The spinal canal is the space in the backbone where the spinal cord is present. The role of the spinal cord is to act as a connection between the brain and the rest of the body. The spinal cord collects signals from the body and sends them to the brain; similarly, it sends the response signal from the brain to the rest of the body. Sensation from the body, movement of the body, autonomic functions, and reflexes all depend on spinal cord function.

The backbone is not made of a single bone but is made up of many bones called vertebrae. These vertebrae form joints with one another with discs in between them. If the back is a single bone, then the spine's movement or flexibility is impossible. Having many bones or vertebrae in the back helps in movement at the back and gives flexibility to the body.

Any break in vertebrae or backbone is called a fracture of the spine or vertebral fracture.

Fracture of spine/ vertebral fracture is commonly caused in patients having osteoporosis or weak bones. Injury/ trauma to the spine causes spine fracture/ vertebral fracture.

Spine fracture or vertebral fracture is divided into following segments:

  • Cervical region or neck – these are vertebral fractures located in the neck
  • Thoracic region or back in rib cage- these are vertebral fractures located at the back of the rib cage from neck to below the rib cage
  • Lumbar region or low back are vertebral fractures at the low back below the rib cage.

Spine fractures/ vertebral fractures are of two types:

  • Stable Fractures- in which the spine vertebrae are fractured but are in such shape and position that the spine is stable. There are fewer chances of surgery in these cases.
  • Unstable Fractures- in which the vertebral fracture is such that the shape or position of vertebrae is abnormal and the spine is not stable.

The risk factors for spine fractures/ vertebral fractures are as follows:

  • Osteoporosis or weak bones
  • Age common in adults above 50
  • Cancer of vertebrae
  • Infection of bone of the spine
  • Vit D3 deficiency
  • Kidney disease

If the spine is stable, the fracture was not caused by severe trauma, and the patient does not have spinal cord injury; the patient can walk but with pain in the back. If you are having pain in the back, which increases on movement or increases over time, kindly consult the surgeon.

It depends on the severity of the injury and the spinal cord injury:

  • Pain in the area of spine fracture/ vertebral fracture
  • Difficulty in walking/ sleeping on back
  • Change of posture
  • Loss of height
  • If the patient has spinal cord injury, then paralysis, loss of bladder and bowel function may be present with other problems of spinal cord injuries

To diagnose the spine fractures or vertebral fractures include tests such as X-rays, MRI, CT scans.

Spine fracture or vertebrae fracture can be treated in following manner:

  • Rest
  • Use of brace
  • Physiotherapy after proper rest
  • Treatment of the cause of fractures like osteoporosis
  • Painkillers use

If treated without surgery, it usually takes three months for treatment of spine fractures or vertebrae fractures. The final time depends on the severity of the injury, age, and other complicating factors.

The surgery for spine fracture or vertebrae fracture is done in the following cases:

  • If after taking treatment for a long time, the pain is still present so much that it affects your daily life.
  • Unstable spine
  • Fractures with spinal cord injuries

A final decision has to be taken in consult with a surgeon

It depends on the cause of injury, association with spinal cord injuries, and type of fracture

  • Spinal cord injuries- decompression and stabilization with screws, rods, and plates
  • Fractures due to osteoporosis or pathological reason

a) Vertebroplasty– this involves injecting bone cement in the vertebrae with the help of a hollow needle called a trocar under fluoroscopy or x-ray guidance.

b) Kyphoplasty- this involves the use of a balloon that is inflated inside vertebrae before injecting bone cement into vertebrae with the help of a hollow needle called a trocar under fluoroscopy.

What, why, and how surgery has to be done has to be discussed with the surgeon before surgery.

Usually within 1-2 weeks if it involves light work or desk job. If the job involves heavy-duty weight lifting, the patient will require six weeks.

If done with proper indication, both vertebroplasty and kyphoplasty have been found to have outstanding pain reduction in osteoporotic vertebral compression fractures.

Vertebroplasty/ Kyphoplasty involves following complications:

  • Cement leak
  • Fracture in adjacent bone
  • Infection
  • Increased pain in the back
