Giant Cell Tumor of Bone

A giant cell of bone is a benign tumor, which means noncancerous in patient terms but has different properties. These tumors grow at the end of long bones. Commonly, they grow at the lower end of the femur (thighbone) or upper end of the tibia (shinbone) close to the knee.

They can also occur in the distal radius (forearm bone near wrist), proximal humerus (arm bone near shoulder), sacral ala(lower back), spine, and bones of hand and foot.

Most commonly affect the age group of 20-40 years.

Giant cell tumors are aggressive locally and can spread to the surrounding area and cause destruction and expansion of bone.

Rarely can they spread to the lungs.

Also, the recurrence rate/ tumor occurring again rate is high.

If a giant cell tumor is seen through a microscope, large cells are seen, which is called a giant cell tumor.

The cause is not known. Also, there is no relation to diet, environmental factors, or trauma. But further studies are going on to know about any hereditary links.

The symptoms or problems with giant cell tumors include:

  • Pain in the area of the tumor, which increases on the movement of the joint and decreases with rest
  • Swelling of the area where there is a tumor
  • Sudden severe pain if the fracture occurs

Through various tests that include x-rays, CT scans, MRI, bone scans, and biopsies.

Complete removal of the tumor is the only treatment. 

  • Curettage + bone grafting /bone cement use + adjuvant therapy (use of high-speed burr, phenol, argon beam laser)
  • Wide excision of tumor and reconstruction (using bone grafts, artificial joint, or combining both)
  • In very few and very severe cases amputations
  • If the giant cell tumor is not operable, then radiation therapy
  • In rare cases, If spread to the lung, it involves Metastasectomy.

Patients with giant cell tumors of bone are good if the tumor is treated early with no complications after surgery, and the risk of returning after treatment is low. In rare cases, giant cell tumors spread to the lungs, but it is still treatable with positive outcomes. The final goal of treatment is to remove all tumors without risk of recurrence.
